Work Order Request Form

To provide you with better service please review the following:

1. Emergency Maintenance Requests - leave your request on voice mail at (214) 520-0031 and our on-call staff will return your call immediately.
2. Non-emergency Service Requests - use the Work Order Request form and send any of the following three options.

   • Fax: (214) 526-8211
3. Message Boxes: - see message boxes at bottom entitled "Notice to Resident" and "Notice to Vendor". Please follow the directions for better service.
4. Written Service Requests
   • Provide authorization to enter your residence if necessary
   • Eliminates miscommunication regarding what was requested or date of the request
   • Facilitates accountability
